Accessing care and support services provided by the Health and Social Care Partnership requires an assessment of your health and social care needs. This is a needs based assessment which determines what care and support would be most appropriate for your particular circumstances.
Traditionally social work has focused on tasks and activities that you cannot do and areas of your life that you need help with. Increasingly we will focus on what you are able to do rather than what you cannot. This will involve looking at positive areas of your life and community that could be of benefit to your well-being – this is what is known as real wealth. This will help to promote your independence and reduce dependence on services.
As part of this process you will be encouraged to take an active role in completing your assessment. We want your assessment to accurately reflect your needs and aspirations.
We will always offer support, advice and guidance in this process through your allocated social worker and our partners in Advocacy.
