Throughcare and Aftercare Services
All Looked After young people who are approaching their 16th birthday and require additional support to navigate through this transition are eligible to support from Throughcare.
Throughcare is the support offered to young people who are Looked After, either living with family in a residential or foster care placement and are on a Children’s Panel Supervision Order. Aftercare is the support offered once they cease to be looked after.
Pathway Assessment
The law says that if you cease to be looked after on or after your 16th birthday, Inverclyde HSCP must make a full assessment of all your needs.
There should be a plan which says how these needs must be met. Areas which are covered in the Assessment and Pathway Plans are:
- Lifestyle
- Family & Friends
- Health & Wellbeing
- Education/ Training & Employment
- Accommodation Needs
- Financial Needs
- Rights & Legal Issues
To do this we will:
- Listen to you
- Acknowledge your views
- Provide emotional support
- Provide practical advice & assistance
- Work with you in defining your identity
- Work with other agencies
- Work to meet your needs
- Acknowledge your life experiences
- Encourage participation
- Advocate on your behalf when required
- Promote choice
We will also help you to:
- Live a healthy lifestyle
- Access leisure and recreational facilities
- Access education, training and employment
- Promote your safety
- Promote equality and recognise diversity
- Challenge discrimination
- Promote your rights and responsibilities
