Inverclyde Community Learning Disabilities Team
Inverclyde Community Learning Disabilities Team is available to assist adults with a learning disability
The service will provide assessments relating to people with learning disabilities social and health needs. The team works with service users from the age of 16 years onwards. Some health services are provided by paediatric services up to the age of 18 and beyond. The team provides advice and support.
An assessment of need is undertaken and from this a support plan is drawn up. These are done in conjunction with service users, their family or carers and anyone who is involved. Information is gathered with consent. For more details about assessments use the link for Assessment of client care.
The service works in partnership with Third Sector Organisations to provide care support services and enable people with learning disabilities to engage within their local communities.
